To Register As A New
Patient |
To register as a patient either phone or visit the practice to check
that you live within the practice area. The receptionists can then
assist you in registering.
All new patients will be asked to complete a health questionnaire and
book an appointment with either a nurse or doctor, within a month of
registering, for a health assessment. This allows us to gain a quick
understanding of any health problems. |
| Obtaining a sickness certificate |
 |
You do not require a doctor's sickness certificate for any
illness lasting 7 days or less. Your employer may, however,
require you to complete a self-certification form which is
available from your employer or at our reception. For any
illness lasting longer than 7 days you will need to see the
doctor for him or her to issue a certificate. |
You may not need to see your doctor in person if we already have a
written report relating to your illness from another doctor, eg a
consultant. Subsequent sickness certificates can sometimes be done over
the telephone but will often require a further appointment to review the
situation.

| Ethnicity |
We are now required by law to collect ethnicity data. The ethnic category
questionnaire will be handed to you to complete when you register with the
practice.

| Spoken Languages |
For patients whose main language is not English, the
following languages are offered by the surgery. French, Hindi,
Italian, Punjabi, Sinhalese, Urdu
Three days prior notification is required to use this service.
The practice also has access to a Face-to-Face and Telephone
Interpreting Service for non urgent appointments.
Please let the receptionist know when booking an appointment, including
the language required.

| Patients aged over 75 years |
All patients over 75 should be seen each year for a health assessment by
either the doctor or a nurse. If you are not able to come to the surgery a visit
can be arranged.

| Well Woman Checks |
| Women of all ages are encouraged to perform self-examination of their
breasts. If you do not know how to do this the nurse will be pleased to show
you. Women aged 50 to 70 will be invited every three years for a mammogram. Women aged 25 to 50 are strongly encouraged to have cervical smear tests
every three years, or more frequently if so advised. Women aged 50 to 64 are
contacted every 5 years.

| Well Man Checks |
Well man checks are carried out by a practice nurse by appointment. Men
of all ages are encouraged to perform testicular self-examination.

| Diabetes, Asthma and Heart Disease |
Patients with these conditions should be seen at least annually by their
doctor and/or nurse.

| Travel Vaccinations |
Our nurses are able to provide a comprehensive vaccinations and advice
service to those venturing abroad on holiday or work. You should contact the
practice nurse well in advance of your travel date.

| Carers |
The receptionist will be pleased to give you an information leaflet if you
look after a relation, friend or child who is disabled, ill or frail.
|
| Training |
The practice is approved for providing training to qualified doctors who are
pursuing a career in general practice. These doctors are with us for up to one
year and have at least three years’ experience in hospital medicine. The
practice is also involved in training newly qualified doctors who are
undertaking a range of posts before deciding on a specialization. In addition,
the practice is involved in the training of medical students. They may sit in
with your doctor. Your permission will be requested for this at the start of
consultation.

| General Practice Associates |
We have a number of general practice associates who are established and
regular members of the practice team. The current GP Associates are Dr De Bono,
Dr Jaulim, Dr Ilves and Dr Ranaweera. Appointments for these doctors may be
offered to all patients registered here and their appointments can be used
interchangeably with those of your registered GP at the surgery. The GP
associates are fully qualified and trained GPs.

Complaints/suggestions |
We constantly strive to give you the best possible care and attention. If you
have any suggestions or are unhappy about any aspect of our service, please ask
to talk to either your own doctor or the practice manager. The practice operates
a formal complaints procedure in conjunction with the PCT.
